Hope Care Connect welcomes individuals, agencies, organizations, nonprofits, churches, camps, events, trade providers, and other care services that want to serve families privately. Choose the application that fits you best. If you are applying as an individual, you must complete the background-check step before being listed. If you are applying as an agency or organization, you will complete a separate review process.

Provider Process

  1. Choose the individual or agency/organization application.
  2. Submit services, experience, service area, and references.
  3. If you are applying as an individual, complete SentryLink and email proof to Hope.
  4. If you are applying as an agency or organization, submit program and verification details.
  5. Hope reviews the application before any listing is added.
  6. Approved providers may receive service requests and payment after completed assignments.
