Background Check Information

What Hope May Review

Written Authorization

If Hope uses a third-party screening company, providers should receive a clear written disclosure and provide written authorization before a background report is ordered. If ongoing checks are needed, that should be stated clearly in the authorization.

Hope should not request a background check until the provider has reviewed the required disclosure and given written consent.

Provider Uploads

Providers may upload proof of a recent background check, but Hope should review the document before showing any public status. Uploaded proof should not replace a new background check when Hope needs current, role-specific, or legally compliant screening.

Provider Background Check Fee

Hope plans to use SentryLink for provider background screening. The provider-facing background check fee is $40.00 and includes the screening cost plus processing and bank charges. Providers should wait for Hope’s instructions and provide written consent before paying for or submitting any background check.

Fair Review Process

Background check information should be reviewed consistently and without discrimination. Decisions should be connected to safety, the role, the service setting, and legal requirements. Medical or genetic information should not be requested except where legally allowed and necessary.

If a Report Affects Approval

Before denying or removing a provider based on a third-party report, the provider may need a copy of the report, a summary of rights, and a chance to respond or dispute incorrect information. Use attorney-reviewed notices.

Family-Facing Labels

Provider profiles should only display background check status if Hope has a reliable process. Suggested labels: “Background check pending,” “Background check completed,” “Credential verification pending,” and “Credential verification completed.” Avoid guaranteeing that a provider is safe.

Provider Photos

Provider photos should be reviewed before publishing. Use clear, professional photos and avoid posting images of children, clients, patients, or private homes without written permission.
